Your First Visit
Dr. Boustany will meet with you face-to-face to discuss your issues. Please be sure to bring a list of your medications, lab results, X-rays, and CT and MRI results. If you prefer, you may drop these off or mail them to us before your appointment. Please download our patient forms and have them completed when you arrive. If you get the completed forms to us prior to your appointment it will speed up the registration process on the day of your appointment.
If you need reading glasses, please bring them with you to your appointment. You may need to complete additional forms during your visit. If we schedule you for surgery, we will ask you to read and sign the necessary consent forms. Bring your insurance or Medicare card as well as a picture ID such as your driver’s license or state issued ID card. Federal law requires us to collect deductibles and copays at the time of service.
During your first visit with Dr. Boustany, he reviews your medical history and conducts an initial medical exam. Dr. Boustany will recommend any further testing, and all treatment options will be discussed in detail.
If you have a life threatening emergency after the office is closed, report to the nearest emergency room or call 911. If it is urgent that you speak with the doctor before the office opens on the next business day, call our answering service at (304) 766-7140
All medication refills prescribed by Dr. Boustany should be handled during regular office hours. If you need a written prescription for mail orders or if you need a refill, please call our office at least 72 hours in advance.
